The Community Placement & Care Navigation Advocate supports seniors and their families in identifying the most appropriate living and care options—such as independent living, assisted living, memory care, or supportive community resources. This role is centered on listening, education, and guidance, helping families make informed decisions without overwhelm or pressure.

Build relationships with Independent Living, Assisted Living, and Memory Care communities.
